For Referrers

Thank you for trusting us with your patients. We endeavor to take the utmost care of our mutual patients and will keep you updated with their progress, returning them to your ongoing care once their treatment with us has been completed.

Please send the following information via email for us to be able to formulate the best treatment plan for our mutual patient.

  • The letter of referral indicating clearly your proposed treatment plan.
  • Recent OPG or other imaging  (for patients requiring implant surgery we’d prefer to arrange the imaging after their initial consult with us).
  • Communication about any other relevant history that will help your patient with their journey at CADS.
  • If a patient is eligible for care in the public system and chooses to pursue treatment in that setting, please refer them there directly.

By prior arrangement and discussion with you, we may be able to see a patient for a procedure at the time of their initial consult (medical history dependent).

Dental issues that warrant an urgent referral

  1. Soft tissue swelling related to an infected tooth. (Patients with fascial-space infections with symptoms such as limited mouth opening and presenting with systemic symptoms such as fever malaise etc. are best referred directly to the nearest tertiary hospital for appropriate and timely management.)
  2. Suspected Malignancy – we would appreciate clinical photographs and a discussion with our specialist clinicians to arrange an urgent appointment.

If you would like to refer through an online referral form please find it here.

Contact us if you would like to request additional referral pads and cards.
